Have you ever went into an Emergency Room and been asked more questions than you care to answer?
Have you ever went into an Emergency Room and been asked more questions than you care to answer especially if you are not feeling well?  Working in a Hospital I see this all the time and have thought there has to be a better way.  I have also been the one answering the questions several times in my 50 years. 

I think I have come up with a way around this especially if you happen to come into the hospital and are unable to talk and you don't have a person with you that can convey this information.
